The North Carolina Healthcare Foundation provides implementation assistance as one part of its process to achieve a NC where quality healthcare is equitable and accessible for all. Through our Implementation Assistance (IA), we drive initiatives from concept to sustainability, aiming to empower healthcare providers and community partners to enhance their operational efficiency, improve patient care, and build sustainable practices that contribute to long-term community health and wellbeing. 

Implementation Assistance

Our approach is grounded in implementation science, with these stages streamlined into the following three key phases of support:

Exploration (3-12 months)

We assess organizational readiness, identify needs, and tailor strategies to set the foundation for success.

  • Site identification and readiness assessment

  • Root causes analysis
  • Patient and provider journey mapping
  • Sustainability planning

Implementation (12+ months, ongoing)

We provide hands-on support to execute the strategies developed in the exploration phase. We work closely with partners to ensure seamless integration of processes and practices, with continuous monitoring and feedback loops to track progress and address challenges in real time.

Sustainability (ongoing)

We help build the infrastructure for long-term success, ensuring that organizations and communities have the resources and strategies needed to sustain improvements independently.

  • Access to actionable data to continue impact reporting
  • Develop long-term communities of practice
